Share Two Current Observations at Care Handover

Whenanother caregiver takes over from you

Share two useful current observations: what the child just needed and what helped, such as “She wanted a quiet corner; the red book helped.” Keep facts separate from guesses and include essential care information through the agreed channel.

Why it works

The next caregiver cannot see the hour you just lived.

Start tiny

Share one observation and one thing that helped.

What you’ll notice

You contribute firsthand knowledge instead of making someone rediscover it.

When it slips

If the handover becomes a report card, remove judgments. Use a brief note when a calm conversation is unavailable. Adapt: Birth–1: current comfort and signals. 1–3: present interest and settling help. 3–6: include child’s own account when welcome.

SafetyShare privately with authorized caregivers. Follow existing care plans; do not invent medical instructions.
